Almost everything we treat used to require an operation. Varicose veins were stripped out of the leg. Fibroids meant a hysterectomy. An enlarged prostate meant transurethral surgery and a catheter. Knee arthritis meant waiting until the joint was bad enough to replace.

Over the last two decades, image guidance changed what is possible. A catheter the width of a strand of spaghetti, passed through a needle puncture and steered under live X-ray, can now reach almost any blood vessel in the body and do something useful when it arrives. The incision disappears. The hospital stay disappears. The months of recovery disappear.

What physician-owned actually means

It means the people making decisions about your care are the people delivering it. There is no administrator setting the length of an appointment, no corporate formulary limiting which device we may use, and no quarterly target that has anything to do with how many patients see us in an afternoon.

It also means we can say no. A center that is paid to do procedures has an obvious incentive to recommend them. Our answer to that is transparency: we will tell you when watchful waiting is reasonable, when medication should be tried first, and when the right referral is to somebody who is not us.

Practically, it means your procedure is performed by the attending physician. Not a fellow. Not a resident. Not a technician working under supervision. The person who explained the procedure to you is the person holding the catheter.

Multi-specialty by design

A vascular problem looks different depending on who is holding the referral. A urologist sees an enlarged prostate as a urologic problem. An interventional radiologist sees it as an arterial one. Both are right, and a patient who only ever meets one of them gets only half the menu.

Our team pairs urology with interventional radiology on the same cases. That is why the same practice can offer prostate artery embolization and the conventional urologic alternatives, and give an honest comparison of both — because we are not limited to recommending the one we happen to perform.

Infection control and the outpatient advantage

Elective procedures done in a hospital share space, air and staff with the sickest patients in the building. Doing the same procedure in a dedicated outpatient suite removes that exposure entirely.

Every procedure at our center is performed using sterile technique in rooms used only for elective, scheduled outpatient work. It is one of the quieter advantages of the office-based model, and it became a great deal less quiet after 2020.
